What the inquiry form collects

Name, email, phone number, state, optional county or equivalent, age range, coverage interest, planning goal, who the prospect wants to protect, existing coverage category, desired amount, timing, optional budget preference, preferred contact method, preferred contact window, time zone, optional reason for the inquiry and a record of the contact request. We also record the submitted source tag and submission time.

How the platform uses it

The platform stores inquiries for authorized team members to review and follow up. It is designed to assign an inquiry only to an agent configured for the visitor’s state. It does not submit insurance applications or issue coverage.

Hiring applications and interview bookings

The hiring form collects name, email, phone, state, licensing status, insurance experience, your reason for applying and a record of your contact request. Agency administrators can review these records, add private notes and manage interview times. A booking records the interview time, interviewer and format. Your browser tab may retain a temporary application access key so you can return to your booking. Public availability does not include applicant details. No hiring decision is made automatically. Calendar files can be downloaded after booking. Confirmation emails and texts are not sent automatically.

Contact choices

The inquiry form requests a personal response. It does not grant permission for automated marketing calls or marketing texts. You can ask the responding agent to stop contact.

For questions about access, corrections, deletion or how long your inquiry is kept, contact lifeagent.joshua@gmail.com. Tell us which request you are asking about without including sensitive identity, medical or payment information.

Information to keep off this form

Do not enter Social Security numbers, payment details, medical records or health histories.
